Forms valid summer 2026

Save time at check-in by downloading and completing your Boat Tahoe forms before your charter. For added convenience, all required forms will also be available on the boat if you prefer to fill them out when you arrive.

All Passengers 18 Or Older

This form is required for all guests onboard. Every passenger age 18 or older must complete and sign their own form. For guests under 18, a parent or guardian can provide the minor’s information in the designated section. We must have a completed release covering every person in your group.

Main Renter – Groups 7+ Only

This form is only required for charters with 7 or more passengers. Groups of 6 or fewer do not need to complete this agreement. Only the primary guest booking the charter needs to fill out and sign this form.

Efoil Specific Charters Only

This form is only required for guests who booked the efoil-specific charter and plan to ride the efoil. Passengers joining the charter who do not plan to use the efoil do not need to complete this form.
